The Optum Support Desk will provide after-hour cellphone emergency support for urgent CernerWorks Citrix portal and CCBH (formerly Anasazi) issues. Urgent issues affecting multiple users, and include:
        -When logging into CCBH (formerly Anasazi), the system does not respond or appears to be frozen, and/or
        -No data can be entered or viewed

The fax number is utilized to:
        -Request stop payment on a check not received via mail
        -Request aging report of unpaid claims for status of claim
        -Send copies of Other Health Coverage (OHC) EOBs if claim is denied
        -Claim Appeals
MFT's may fax or call for MediCal Eligibility Verification to confirm the client's eligibility for the month of service.

         samples for TERM.

Child Welfare Services staff can send in authorization requests and treatment and evaluation referral forms.
Juvenile Probation may send in evaluation referral forms and copies of the Juvenile Court Minute Orders.
